There are two versions. The first was released in September 1998 under Series 12. An exclusive repaint appeared two years later.
September 1998 Series 12 Original
The original version has a body that is tan on the right and green on the left.
If the contradictory Zombie Spawn is not taken into account, this should be the first really bad Spawn and also that of a man-made Spawn. The word re-animation has an origin that dates back to the 16th century but it is H.P. Lovecraft that immortalises it with Dr Herbert West and his re-animators in 1922. That Lovecraft hates his own story is immaterial, it helps to create a re-animated Spawn!
November 2001 Series 12 Diamond Exclusive
The last time Diamond has a spawn exclusive was with the June 1996 Spawn 1 aka 'Black Wormhead Spawn' and this repainted Series 12 seems to be taking its own sweet time too, first released in September 1998, it finally got a repaint only in November 2001. The green part of the body remains but the tan part is replaced by blue. Card image courtesy of Jin119kg's Collection.
This figure is...complicated. The details are so elaborate and yet so...strange.
That theme of reanimation is there for all to see.
The pair of giant wings is even adorned with skulls. And carried by a weird looking contraption with more skull symbol.
There is little doubt hat he is pretty evil looking, what with that skull he carried with him. Incidentally, that head is Overtkill. So yes, in his elaborate complicated design, he is pretty ugly and scary and then...
we have that underwear...
